Submitting your note in A to Z

Most denied leaves are paperwork problems, not medical ones. Here is what each screen does, described from a recorded walkthrough with an Amazon associate.

Choose the leave type

For your own illness, the list offers "My own serious injury or illness." Amazon uses that wording for any medical leave, including short ones — it is the category label, not a judgment about how sick you are.

The short-term disability question

The first question asks whether you want to apply for short-term disability. Answering yes moves your case into Amazon's disability track, which asks your doctor for a Physician Statement — diagnosis codes, medications, and a finding that you cannot work at all. SickSlip does not complete that form.

Continuous or intermittent

Continuous covers days in a row with a start and end date. Intermittent covers a recurring condition, such as days spread over months, and Amazon asks for frequency and duration on that path.

Your dates

Last day worked, the first day of your absence, and your expected return date.

Upload your documents

Five files at a time, and you can add more after submitting. Your SickSlip email carries three files: the note, the E103, and the E117. Amazon's own screen warns that missing documents can lead to leave denial, an attendance violation, or termination.

What gets leaves denied

Upload all three. The note on its own is usually not enough.

Keep them as files. Save the attachments rather than screenshotting them.

Extensions need their own set. Extending a leave starts a fresh documentation request. The screen calls a document optional; the denial letters do not.

Pending is not approved. A quiet schedule is not a decision. The email from DLS is.

Answer when DLS calls. They call to fix paperwork before your deadline runs out.
