SickSlip operates a documentation-focused digital platform designed to facilitate licensed clinician review of absence-related medical documentation. SickSlip: • Does not provide diagnosis • Does not provide treatment • Does not prescribe medications • Does not replace primary care • Does not provide emergency services If a clinician–patient relationship is formed, it exists solely between you and SickSlip P.A., not SickSlip, Inc. If you are experiencing a medical emergency, call 911.

You must: • Be 18 years of age or older, OR a parent or legal guardian submitting on behalf of a minor • Be physically located in a U.S. state where SickSlip operates at the time of submission • Provide accurate and truthful information Submission of false or misleading information may result in account suspension or permanent termination.

Clinical documentation may be issued via asynchronous telehealth review. Telehealth consent is governed by a separate Telehealth Informed Consent document. By using the Service, you acknowledge: • You have reviewed the Telehealth Informed Consent • You understand the limitations of asynchronous review • You may withdraw consent prior to issuance of documentation

HIPAA applicability depends on the nature of the services provided and the role of each entity. If SickSlip P.A. is acting as a HIPAA-covered entity and your information constitutes Protected Health Information ("PHI"), the Notice of Privacy Practices governs the use and disclosure of PHI. For information that is not PHI, or where HIPAA does not apply, the SickSlip Privacy Policy governs. SickSlip operates on a cash-pay basis and does not bill insurance.

The SickSlip platform may use artificial intelligence tools to: • Structure intake information • Categorize submissions • Detect potential fraud patterns • Support workflow routing AI tools do not independently diagnose, treat, or prescribe. All documentation decisions are reviewed and approved by a licensed clinician prior to issuance.

All services are cash-pay only. Standard documentation request: $29.99. Rush processing (delivery in under 10 minutes): $37.99 ($29.99 + $8.00 rush fee). Prices may be changed at any time. Payment is processed through third-party payment processors. Service limitations: Maximum 3 documentation requests per account. Maximum absence coverage per note: 5 consecutive days.

Refund eligibility: Automatic Refund • Request denied prior to clinician review • Technical failure preventing document delivery • Employer or school rejects documentation issued by SickSlip (100% money-back guarantee) No Refund • Documentation issued consistent with submitted information and accepted by employer/school • User provided inaccurate or fraudulent information • Fraudulent use or intent to deceive an employer or educational institution Discretionary Refund • Platform error • Clinician clerical error Note Modifications • Modification requests must be submitted within 72 hours of purchase. • Requested dates must fall within or adjacent to the original absence period. • Total absence period cannot exceed 5 consecutive days. • Modifications are subject to physician discretion at no additional charge. Refund requests must be submitted within 30 days to support@sickslip.co.

To the maximum extent permitted by law: SickSlip's total liability shall not exceed the greater of: (a) The amount paid by you in the prior six months (b) $100 We are not liable for: • Employer or school decisions • Lost wages • Employment consequences • Indirect, incidental, or consequential damages

SickSlip is an independent telehealth medical practice. We are not affiliated with, endorsed by, or sponsored by any employer, health plan, or third-party administrator referenced on our website or in our services. References to specific employers (e.g., "Amazon"), employer leave programs (e.g., "Disability and Leave Services"), or specific employer leave forms (e.g., "Health Care Provider Form (E103)" or "Authorization to Obtain and Disclose Information (E117)") are descriptive only — used solely to identify the documents an employee has been asked to submit by their employer. Such references are nominative fair use and do not imply any partnership, endorsement, certification, or business relationship with the named entity. These Terms are governed by the laws of the State of Delaware.
