Our physicians issue documentation only when symptoms and circumstances support it clinically. If the reviewing physician determines a note isn't appropriate for your case, you'll receive a full refund and a brief explanation. What kinds of illnesses qualify?

Common, short-term, non-emergency conditions are typically eligible — for example, colds, flu, stomach bugs, migraines, and back pain. The reviewing physician evaluates each case based on reported symptoms and circumstances and may request additional information before issuing documentation. Notes are not guaranteed and are issued only when clinically appropriate. We do not cover extended leaves (FMLA / disability), legal proceedings, medical clearance for activities, or general personal time off.

Will my employer accept this?

SickSlip notes meet standard medical documentation requirements: a board-certified physician's signature, NPI, state license number, and a QR verification code. Employers can scan the QR code or call our verification line at (877) 861-4165 to confirm authenticity directly with our office. If your employer has questions, email support@sickslip.co and we'll help facilitate verification. As with any medical documentation, acceptance is ultimately at each employer's discretion.

Yes. Select "School" during the intake and the note will be formatted accordingly. A parent or guardian can submit on behalf of a child.
