SickSlip operates a documentation-focused digital platform. SickSlip is structured as follows: • SickSlip, Inc., a Delaware corporation, provides non-clinical technology and administrative services. • SickSlip P.A., a Florida professional association, is owned by a licensed physician and provides professional medical review and documentation services.

Information You Provide: • Name, date of birth, email address, phone number • State/location at time of submission • Absence dates and description of illness • Communications with customer support Payment Information: Payments are processed by third-party payment processors. We do not store full payment card numbers. Automatically Collected: • IP address, device and browser type • Access times and pages viewed • Approximate geolocation derived from IP

We use personal information to: • Provide and operate the Service • Enable licensed clinician review • Deliver documentation • Process payments • Prevent fraud and abuse • Comply with legal obligations • Advertise our services and measure how well our advertising works (see Section 4)

We advertise on platforms such as Google, Microsoft, Meta and Reddit, and we use analytics tools to understand how people find and use our site. To do this, we may: • Use cookies and similar technologies on our public pages so these platforms can measure visits and purchases that come from our ads. • Share limited contact details, such as your email address or phone number, with advertising platforms in scrambled (hashed) form. They use this to recognize existing customers, to avoid showing them ads they do not need, and to find new people who may be interested in our service. What we do not share with advertising platforms: your medical information, the reason for your request, your absence dates, or your documentation. We do not sell your personal information for money. Your choice: to stop your contact details being used for advertising, email support@sickslip.co with the subject "Opt out". We will remove you within 10 business days. You can also control cookies in your browser settings.

We implement administrative, technical, and physical safeguards including: • Encryption in transit • Encryption at rest (where applicable) • Role-based access controls • Multi-factor authentication No system can guarantee absolute security.
