Every note is personally reviewed and signed by a board-certified physician licensed in your state. Each note displays the physician's NPI number, state license information, and includes a unique QR verification code so the document can be authenticated. If the reviewing physician determines a note isn't clinically appropriate, we provide a full refund and a brief explanation.

Our physicians issue documentation only when symptoms and circumstances support it clinically. If the reviewing physician determines a note isn't appropriate for your case, you'll receive a full refund and a brief explanation. Clinical judgment is non-negotiable — that's what makes our notes legitimate medical documents.

Will my employer accept it?

SickSlip notes meet standard medical documentation requirements: a board-certified physician's signature, NPI, state license number, and a QR verification code. Employers can scan the QR code or call our verification line at (877) 861-4165 to confirm authenticity directly with our office. If your employer has questions, email support@sickslip.co and we'll help facilitate verification. As with any medical documentation, acceptance is ultimately at each employer's discretion.

What kinds of illnesses qualify?

Common, short-term, non-emergency conditions are typically eligible — for example, colds, flu, stomach bugs, migraines, and back pain. The reviewing physician evaluates each case based on reported symptoms and circumstances and may request additional information before issuing documentation. Notes are not guaranteed and are issued only when clinically appropriate. We do not cover extended leaves (FMLA / disability), legal proceedings, medical clearance for activities, or general personal time off.

Will SickSlip work for an Amazon FMLA leave or 'Serious Health Condition' form?

Accepted Answer

No. The Amazon Healthcare Provider Form is part of Amazon's FMLA / Leave of Absence process and requires a treating physician relationship and a formal Serious Health Condition certification under federal FMLA law (29 CFR 825.306). The U.S. Department of Labor specifically requires a video visit (not asynchronous review) to satisfy FMLA's clinical evaluation standard. SickSlip is an asynchronous service for routine short-term absence notes only — we are not able to issue FMLA documentation, complete the Amazon Healthcare Provider Form, or modify an existing note to add diagnosis or symptom information. If your Amazon HR team is requesting any of those, you'll need to work with your primary care physician or a telehealth service that offers scheduled video FMLA visits.
