1. Nature and Scope of Services
SickSlip operates a documentation-focused service designed to facilitate licensed clinician review of absence-related medical documentation.
The Service is for short-term, acute, non-emergency illness — typically a 1–5 day absence.
The Service:
• Does not provide diagnosis
• Does not provide treatment
• Does not prescribe medications
• Does not replace primary care
• Does not provide emergency services
• Does not complete FMLA, short-term or long-term disability, or extended or intermittent leave certifications, and does not certify a "serious health condition" as defined under the federal FMLA. These require an ongoing treating-provider relationship and, where applicable, an in-person or synchronous evaluation that this asynchronous service does not provide.
• Does not provide fitness-for-duty, activity or medical clearance, or accommodation certifications beyond a routine return-to-work date
• Is not for serious, chronic, or emergency medical conditions
Documentation integrity: SickSlip documentation reflects the licensed clinician's independent judgment based on the symptoms and information you report. SickSlip does not issue documentation stating a diagnosis, condition, severity, or date that your submitted information does not support, and will not do so to help you obtain a benefit, leave determination, or outcome for which you do not otherwise qualify. Requesting documentation of a condition you do not have is prohibited and may result in denial without refund and account termination.
Dates: You may request documentation covering a past absence window, but each document reflects the actual date of clinician review — the clinician's signature and review timestamp are not backdated or altered.

2. Asynchronous Telehealth Services
Telehealth services on the SickSlip platform are typically delivered asynchronously.
This means:
• You submit information electronically through the platform.
• A licensed clinician reviews your submission at a later time.
• The clinician may approve, request clarification, or decline issuance of documentation.
• There is no guarantee of approval.
• A real-time video or audio visit is not required.
3. Limitations and Risks of Telehealth
Telehealth services involve inherent limitations, including:
• The absence of a physical examination.
• Reliance on the accuracy and completeness of the information you submit.
• Potential delays due to technology disruptions.
• The possibility of unauthorized access to electronic communications despite reasonable safeguards.
4. Withdrawal of Consent
You may discontinue use of the platform at any time prior to issuance of documentation.
Submission of a request constitutes consent to asynchronous telehealth review for documentation purposes.
Withdrawal of consent after documentation issuance does not invalidate documentation already issued.

We do not send SMS/text messages to patients. If you provide a phone number on the intake form, it is used only for support callback if we need to reach you about your order. Providing a phone number is optional.
