The short answer

Why strep is different from a cold

Most sore throats are viral and resolve on their own. Strep throat is bacterial — caused by group A Streptococcus — and it has a few important differences:

• Voice changes (muffled, 'hot potato' voice) — abscess concern
• Stiff neck or severe headache — rare complications worth evaluating
• Symptoms persisting or worsening 48 hours after starting antibiotics — wrong antibiotic, wrong diagnosis, or developing complication
• Rash that spreads from the trunk outward — could be scarlet fever, which is strep-related and benign but worth confirming
